timely manner.  So, whether it be purchase, maintenance or events request forms, you will find
them right here.  So, please click on the button below to the form you need and submit your
request. Each request  is discussed and looked over during Staff Meeting on Tuesday's. After it
has been discussed you will receive an email pertaining to your request. 

Maintenance Request Form

Event Request Form

Have an idea for an event you would like to submit? Great! Please fill out this form so our staff can look over it and make sure the dates are open, funds are available, and that the event meets the vision of MAG Church. Thanks!

Purchase Request Form

This form is for purchases authorization for items on behalf of MAG Church. Please be advised that this form is only needed to be filled out if the sum total of an item is above $50. Please allow up to 5 days to receive an answer to your request.
